Description This module will prepare students for physiotherapy practice working with people with long term conditions in community-based settings. This is a major area of physiotherapy practice and pre-registration physiotherapy students are typically allocated at least one practice-based placement in these settings during the programme.

Students will build upon foundational knowledge acquired during prior study and further developed in Semester 1 of the programme as they develop common principles and core skills in the support and management of people with long term health conditions that may then be critically applied when working with diverse populations in a range of clinical contexts (e.g. multiple sclerosis, chronic low back pain, cerebral palsy, chronic obstructive pulmonary disease and dementia). The International Classification of Functioning, Disability and Health (ICF) will be used to provide a framework for clinical reasoning as students develop their ability to generate and critically analyse evidence-based inter-professional management approaches for people with long-term conditions.

The development of related knowledge and skills will be facilitated by a case-based learning approach built around scenarios which reflect the complexity of current clinical practice. The module has an emphasis on fostering the student’s problem-solving and self-directed learning skills through engaging active-learning approaches and a substantial practical component including clinical simulation and service users.

Learning Outcomes Students will be able to:
  • Synthesise and apply knowledge and understanding of common problems experienced by people with long term conditions in community settings.
  • Critically apply the International Classification of Functioning, Disability and Health (ICF) in the formulation of clinical reasoning to support the provision of evidence-based person-centred care.
  • Synthesise research evidence with person-centred clinical reasoning to provide and critique safe and effective physiotherapy practice for people with LTCs in community settings.
  • Critically analyse adaptations practice required to ensure equitable care for people with LTCs in a range of personal and social contexts encountered in community settings. (Assessed by both modular and synoptic assessment)
  • Critically analyse adaptations to practice required to ensure equitable care for people in a range of personal and social contexts encountered in physiotherapy practice. (Assessed by both modular and synoptic assessment)
