Students will work in small groups (approx. 5-6 students), Each group will select a clinical case/patient about which to write:
A summary/report that includes a patient profile, the man features of the case, the implications of the case for on-going self-management;
Information for use by the patient and/or clinical colleagues, for example a GP referral letter, an appropriate discharge letter/plan and a patient information leaflet about self-management of their condition
Working online, each group will review and provide feedback on the work of a second group, noting what was done well and what could be improved.
Following this, trained clinically qualified facilitators will assess the work and feedback provided by each student group, noting whether it has been undertaken with reasonable diligence and providing formative comments on both their work and their peer review.
Learning Outcomes
By the end of the module students should be able to:
Demonstrate an understanding of the issues relating to patient self-management of chronic conditions. This will include the importance of ensuring that patients understand:
a) their condition and how to manage it from day to day;
b) how to recognise an emergency and what to do about it.
Demonstrate an understanding and awareness of:
a) Quality standards for written information between colleagues
b) Quality standards for patient information
c) A range of tools, templates and resources that will support the preparation of high quality written information
d) Different approaches to giving feedback to colleagues.
Write, to an appropriate standard, written information for use by colleagues and patients, for example:
a) a referral letter from primary to secondary care
b) a discharge letter for an acute condition with long term chronic implications
c) an information leaflet for a patient who will need to manage their chronic condition on a day to day basis
Assess the quality of written information prepared by another student (colleague).
Write accurate and constructive feedback on the quality of written clinical information prepared by another student (colleague).

Assessment Methods & Exceptions
Assessment:
Satisfactory completion of tasks by the group will be required. Each individual will be required to confirm that they have contributed to all sections of the component and that they agree to submission of the work. If a group’s work is unsatisfactory, each student in the group will undertake the tasks again, this time on an individual basis. Time allowed for completion of this resit loop will be approximately one month.
Reassessment:
If a group’s work is unsatisfactory overall, each student in the group will undertake the component tasks rated unsatisfactory again, this time on an individual basis. Time allowed for completion of this resit loop will be approximately one month.
