This module will prepare students for physiotherapy practice working with people with long term conditions in community-based settings. This is a major area of physiotherapy practice and undergraduate physiotherapy students are typically allocated at least one practice-based placement in these settings during the programme. Students will build upon foundational knowledge developed in Year 1 of the programme as they develop common principles and core skills in the support and management of people with long term health conditions that may then be applied in a range of clinical contexts (e.g. multiple sclerosis, chronic low back pain, cerebral palsy, chronic obstructive pulmonary disease and dementia). The International Classification of Functioning, Disability and Health (ICF) will be used to provide a framework for clinical reasoning as students develop a coherent and consistent approach to considering the inter-professional management of patients with long-term conditions.
The development of related knowledge and skills will be facilitated by a case-based learning approach built around scenarios which reflect the complexity of current clinical practice. The module has an emphasis on fostering the student’s problem-solving and self-directed learning skills through engaging active-learning approaches and a substantial practical component including clinical simulation and service users.
Learning Outcomes
By the end of the module students should be able to:
Demonstrate knowledge and understanding of common problems experienced by people with long term conditions in community settings.
Apply the International Classification of Functioning, Disability and Health (ICF) in the formulation of clinical reasoning to support the provision of person-centred care.
Demonstrate practical knowledge and application of holistic clinical skills appropriate to the physiotherapy assessment and management of people with a range of long term conditions.
Demonstrate knowledge and understanding of the role of the physiotherapist in the interprofessional management of people in community settings.
Demonstrate evidence-informed, person-centred clinical reasoning to provide safe and effective physiotherapy practice in community settings.
Show an ability to adapt practice to varying personal and environmental contexts that may be encountered in community settings.
